The ward is a solution for patients who, after the acute phase of hospitalization, require an extended hospital care intervention due to a compromised functional situation from which may arise conditions such as clinical instability, reduction of recoverable self-sufficiency through rehabilitation interventions, or the need for continuous nursing assistance.

Post-acute Long Stay assists non-self-sufficient patients from various medical or surgical care areas in an inpatient setting. The multidisciplinary team plans and manages the assistance and rehabilitation treatment for patients with an unstable and persistent clinical condition characterized by functional limitations, chronic disability not stabilized, or in a terminal phase.

Inpatient care also allows for the identification of subsequent and more appropriate care pathways.

Rehabilitative interventions focus on functional recovery, prevention, and treatment of degenerative processes.

The facility ensures continuous medical monitoring 24/7, nursing care, and rehabilitation treatment. The standard duration of the assistance and rehabilitation treatment is typically 60 days from admission to the facility.

The ward provides specialized consultations in cardiology, urology, neurology, internal medicine, and ophthalmology, as well as any others deemed necessary for the patients.

Accreditation

Access is granted upon request, using specific forms, from the physician of the hospital unit of origin to the Admissions Office and CUP. The Admissions Office and CUP promptly communicate bed availability to the hospital unit of origin and inform the relevant ASL offices.

At the time of admission, the following documentation must be presented:

a) Copy of a valid identification document for the user;

b) Copy of the Health Card (TEAM, STP, ENI, …) of the user;

c) Any decree appointing Administrator of Support/Tutor and a copy of the identification document;

d) Hospital discharge letter;

e) Any other documentation that the department may deem necessary to request.

CLOTHING:

  • 4 intimate shirts or undershirts;
  • 4 regular t-shirts;
  • 6 underwear;
  • 3 nightgowns or pajamas;
  • 6 pairs of socks;
  • 4 tracksuits;
  • 4 face towels;
  • 4 bidet towels;
  • 2 bath towels;
  • 6 cloth napkins;
  • Sneakers or closed slippers.

For wheelchair users, a pillow (matching the size of the wheelchair and NOT anti-decubitus) is recommended.

ACCESSORIES FOR PERSONAL HYGIENE:

  • Tissues;
  • Disposable cups;
  • Liquid soap;
  • Shampoo;
  • Shower gel;
  • Sponge;
  • Comb;
  • Electric razor;
  • Zippered case;
  • Toothbrush and toothpaste;
  • Denture tablets (for denture wearers);
  • Denture case (for denture wearers).

Life in the ward:

As a precautionary measure to contain Covid-19 contagion, visits are possible at the moment by appointment arranged with the ward.

Doctors meet with patients' relatives by appointment. All appointments must be made with the Nursing Coordinator of the department.
